Project Coordinator, Chronic Conditions

As the Project Coordinator, Chronic Conditions you will be part of the Commissioning team, reporting to the Manager, Service Development and Performance - Chronic Disease.

You will support the strategic initiatives of this portfolio focusing on the implementation of agreed activities and projects aimed at managing care across the health risk continuum including prevention, low risk, rising risk and high risk and improving health outcomes for people across the SENSW region. Additionally, the role is required to work in close collaboration with internal stakeholders to support the performance of chronic disease initiatives engaged in Primary Care.
