COORDINARE is on the lookout for a passionate and dedicated Commissioning Manager, Chronic Disease to join our team and report to the Director, Community and System Collaboration.
Purpose for this role:
You will collaborate with consumers and service providers to ensure best practice commissioning of chronic disease prevention, treatment and management initiatives which achieve:
- improved health outcomes,
- reduced inequalities and better patient experiences across the across South Eastern NSW region.
